Medication Reconciliation Assignment

While reviewing my client’s medications that he has been taking I did notice a few safety concerns. The main safety concerns that we are concerned about is the adverse effects of his medications. The first being is monitoring for hyperkalemia because of the clients’ use of Losartan. Hyperkalemia is a main adverse effect of this medication, and this electrolyte needs to be monitored to prevent any arrythmias in the future. Another safety concern that we need to be aware of is the adverse effects of Amlodipine. Since this drug causes orthostasis and lightheadedness, it poses a safety risk for my client since he takes his medications in the morning before he goes to work because it puts him at risk for lightheadedness while driving which can cause accidents. We also highlighted that while taking Atorvastatin he is at risk for muscle pain and cramps which is a safety concern for my client because he has a very hard-working job that is demanding on his body. So, with these adverse effects from this medication it can lead to an increase of the effects which can lead to major pain and dysfunction. These safety concerns are not related to the age of my client, but they were related to his knowledge and understanding of these medications’ adverse effects. The client was also aware of why he was on these medications, but he was not aware of some of the adverse effects that can come with each medication. The client’s ability to know why he is taking these medications is very important because you want the client to be aware of what is going on in their body and that is why they need these medications.

There were two medications that have a similar action. Losartan and Amlodipine are both blood pressure lowering drugs. The client was aware of why he was on both of these medications and that is to lower his blood pressure. The only medication interaction that I reviewed with my client was him taking Antacids or calcium carbonate with all of his other medications. I informed him that he should try to take this medication by itself because it can interfere with his other medications absorption rates.
